If you do not want to send your SS# via email, please contact the Admin office by phone (714-897-4026) to submit your SS#.  This information is needed BEFORE your request can be submitted for review.

Submission of a request does not guarantee approval.  Your request will be reviewed & you will be notified whether it has been approved or not.  Please make any inquiries about that status of your request through the Admin office (714-897-4026).  Thank you.
